Accepted Documentation

You’ll need to attach supporting documentation to your application:

• A current and valid Access Card including the queuing logo on it. These cards are available here (www.accesscard.org.uk)
• Disability Living Allowance (dated within the last 24 months) showing that you receive either the higher rate mobility component or the high rate care component. 
• PIP letter (dated within the last 24 months) showing you receive the enhanced rate for either daily living or mobility components.
• A current and valid Blue Badge.
